Please follow the steps below to submit your name change request.
How to Request a Name Change
Current Students
- Login: Click the “LLU Student Login” button to access the Name Change Request form. Your current name will be automatically pulled from the system.
- Complete the Form: Select the reason for the name change and enter your new name as it appears on the supporting documentation.
- Provide Documentation: Upload the necessary legal document supporting your name change (e.g., certified marriage certificate, court order).
- Submit Request: Click the “Submit” button to send your request for processing.
Alumni
- Login: Click the “Previous Student Login” button to access the Name Change Request form.
- Complete the Form: Provide the name you used while in attendance, enter your current name as it appears on the supporting documentation and select the reason for the name change.
- Provide Documentation: Upload the necessary legal document supporting your name change (e.g., certified marriage certificate, court order).
- Submit Request: Provide your signature and click the “Submit” button to send your request for processing.
Processing Time
Please allow up to 5 business days for your request to be processed. You will receive a confirmation email once your name change has been updated in our records.
