How to Request a Name Change

Current Students

  1. Login: Click the 鈥淟LU Student Login鈥 button to access the Name Change Request form. Your current name will be automatically pulled from the system.
  2. Complete the Form: Select the reason for the name change and enter your new name as it appears on the supporting documentation.
  3. Provide Documentation: Upload the necessary legal document supporting your name change (e.g., certified marriage certificate, court order).
  4. Submit Request: Click the 鈥淪ubmit鈥 button to send your request for processing.

Alumni

  1. Login: Click the 鈥淧revious Student Login鈥 button to access the Name Change Request form.
  2. Complete the Form: Provide the name you used while in attendance, enter your current name as it appears on the supporting documentation and select the reason for the name change.
  3. Provide Documentation: Upload the necessary legal document supporting your name change (e.g., certified marriage certificate, court order).
  4. Submit Request: Provide your signature and click the 鈥淪ubmit鈥 button to send your request for processing.

Processing Time

Please allow up to 5 business days for your request to be processed. You will receive a confirmation email once your name change has been updated in our records.
